Histopathological Study of Renal Needle Biopsy Specimens In Cases of Upper Urinary Tract Calculi


Abstract
Calculus of the upper urinary tract is one of the most common diseases causing urinary obstruction. Disturbance of renal function caused by urinary obstruction results in pyelonephritis mainly due to stasis and bacterial retention. Pyelonephritis and calculi aggravate and accelerate the disease process toward renal failure.
The recurrence of urinary calculi is also quite common. The author performed 30 cases of the renal needle biopsy among the patients of upper urinary tract calculi, when surgical intervention was made, for the investigation of the rate of coexistence of above factors.
1. 21 males and 9 females were studied excluding renal tumor, renal cyst, severe hydronephrosis and the patient with the hemorrhagic tendency. The range of age were between 10 to 59 years.
2. The calcification in kidney was found in 13 cases (43%) through careful histologic search of biopsy specimens prepared with hematoxylin and eosin staining method.
3. The histopathological examination revealed 8 cases (27%) of acute pyelonephritis, 22 cases(73%) of chronic pyelonephritis, which includes 2 cases of active type, 15 cases of intermediate type and 5 cases of healed type.
4. Histologic study was also made to observe correlation between microscopic calculi (calcification) and pyelonephritis and calcification was found in 2 cases out of 8 acute pyelonephritis cases, in 7 cases out of 15 intermediate type, in 4 cases out of 5. healed type of chronic pyelonephritis, respectively. The calcification seems to be found more often in the chronic type of pyelonephritis than in the acute type.
